The film used a variety of methods to portray Chucky, including RC animatronics and little people or child actors. Multiple animatronics and cosmetics were used in every scene throughout the film, with Chucky's cosmetics shifting from a toy-like look to a more human one. The production created several Chucky animatronics, such as a tantrum-throwing Chucky, a walking Chucky and a motionless Chucky. The animatronic face was controlled remotely through a rig worn on a person's face that recorded their facial movements.

The first draft of the screenplay was completed in the summer of 1985 and featured Chucky under the name Buddy, who comes to life after mixing blood with Andy and kills those who wrong Andy (such as the babysitter and the teacher), exposing Andy's loneliness and isolation from an overworked mother and an absent father. Besides being a more psychologically focused horror film, "Buddy" only came to life at night while Andy slept. Several scenes from later sequels originated in this draft, including the death of Andy's teacher, the climax in a toy factory (Child's Play 2), the ending in which blood mixes with molten plastic and begins to swirl (Child's Play 3), and Andy's teenage babysitter being electrocuted (Bride of Chucky). Buddy was supposed to have a pull string on his back in order to speak, and when he later revealed himself to be alive, he would pull it further and further down his back until his voice deepened and he tried to strangle Karen with the string.

All of Brad Dourif's voice work for Chucky was recorded in advance so that Chucky's mouth could be matched to the words. For this reason Dourif rarely appeared on set during the doll scenes; instead, recordings of his voice were played so that Alex Vincent could react to them. Dourif did, however, perform the doll's movements during rehearsals, as seen in the making-of videos included on the DVD.

In the DVD commentary it was noted that Chucky's head is cut off as he approaches Karen and Andy not because Karen blows it off, but because Mike quickly enters the frame and decapitates him with a baseball bat. If you slow that scene down, you can actually see a bat enter the frame and take off Chucky's head.

Maggie's death scene originally had her being electrocuted while taking a bath. Don Mancini changed her death to being thrown out of a window because he thought it would be more shocking and sardonic. Her original death was reused for Tiffany's electrocution in Bride of Chucky (1998).

According to an interview with Brad Dourif, who voices Chucky in all the films, his daughter Fiona, who was 7 years old at the time, came into the recording room during a session. He was recording Chucky's agonized scream for the scene in which Chucky is set on fire in the fireplace. No one, not even Dourif himself, knew his daughter was in the room with him until her screams from the recording frightened her and she began to cry.

According to director Tom Holland, the scene in which Maggie falls to her death was achieved by having a stuntwoman perform the main shot and then dropping a dummy filled with concrete that hit the car, beneath whose windows were explosives set to shatter the glass.

During the first screening, a crowd of protesters gathered around the entrance of Metro-Goldwyn-Mayer (MGM) calling for the film to be banned. They claimed it would encourage violence in children. Local news reporters were broadcasting live from the scene, and producer David Kirschner watched, disturbed by what he saw. Jeffrey Hilton, who worked with Kirschner at MGM, said he could defuse the situation within ten minutes. Hilton went down and spoke with the ringleader, after which the group dispersed, to the dismay of the news anchors. Hilton never revealed whether it was a threat or diplomacy that saved the day.

Don Mancini stated that Chucky was primarily based on the My Buddy dolls: "In my original script he was named Buddy and we couldn't use it because of the My Buddy doll. The director went out and bought a My Buddy doll, a Raggedy Ann, a Raggedy Andy and one of those life-sized dolls. I told [designer] Kevin Yagher that I wanted something like the My Buddy doll. In my original script I had described "Buddy," now "Chucky," as having red-buttoned overalls, red sneakers, a striped sweater, red hair, blue eyes and freckles. Kevin went and drew many designs of Chucky until the final one was chosen. Yagher then made the first doll from these drawings and my details."

According to the shooting script, Andy's father was named Bob and had died in a car accident a few months before the events of the film. A deleted scene would have removed the debate surrounding the mystery of how Chucky knew the family's history. During the birthday celebration, Andy was giving his toys a tour of his room to show them to his new friend. He would also have shown Chucky a photo of his family and commented on how much he still missed Bob. Apparently Chucky used the boy's emotional attachment to his father to persuade Andy to follow his plans. In another deleted scene, Andy was talking with Karen about where his father was. These scenes never made it into the final cut.

In a September 2008 interview, Don Mancini and David Kirschner discussed a remake rather than a sequel because fans wanted to see a scary Chucky film again, not a comedic one like the last two films in the series. If the remake had been made, Brad Dourif would initially have returned as the voice of Chucky because Mancini believed no one else was suited to the role. In a later interview, Mancini described the script as darker and scarier than the original film; the surprises and twists were not far removed from the original concept. This remake idea later became Child's Play (2019), though without Dourif or Mancini involved.

The film's original working cut ran over two hours. Other sources claimed the original cut ran two and a half, nearly three hours. Some of the deleted or cut scenes include: 1. The original opening in which Mike dresses in women's clothing to catch Charles Lee Ray (if you watch the normal opening scene closely, you can see Mike throw the dress to the ground while chasing Ray down the street). 2. Andy showing Chucky his room. 3. Andy falling into a ditch during the explosion of Eddie Caputo's hiding place. 4. Chucky's death, in which his friend John/Dr. Voodoo performs a ritual dressed as a priest. 5. Andy befriending a young girl during his stay in the mental hospital; Chucky later tricks the same girl into helping him find Andy. 6. Footage of Chucky following Andy in the mental hospital. The DVD commentary also mentions an alternate ending in which Mike kills Chucky by decapitating him with a baseball bat. The deleted scenes of Child's Play (1988) have never surfaced online. Director Tom Holland says the scenes were in his script and that he shot them, but they were removed while cutting the film.
