In the original film, Jade was 15 and David was 17, and they began dating after her brother Keith introduced them to each other; a subplot involved Ann falling in love with David and, after watching them have sex one night, living vicariously through him and Jade. David's surname was also Axelrod, changed here to Elliot, and Ann's name is spelled Anne.

There are many differences between the original and this remake. In Endless Love (1981), Keith hated David and hated the idea of David and Jade being together as a protective older brother. In this remake, Keith treats David far better and more helpfully, accepts him as his sister's boyfriend and sees him as the perfect man for her. In the original, it was Keith's actions that ruined David and Jade's relationship; here, in a sense, it is thanks to him that David and Jade end up together.

Both this version and the 1981 version differ significantly from the novel they are said to be based on. Spencer's 1979 'Endless Love' had many fans, and many were shocked to see the film version take a story involving mental illness, family dysfunction, stalking and destructive obsession and turn it into a teenage romance with a happy ending. In the novel, David is mentally ill, Jade is shallow and aimless, the Butterfields are falling apart, and the ending is not happy at all.
