Several clues suggest the film's opening is a simulation: the therapist clicks her pen, which seems less a habit than an attempt to hypnotize Rourke; the elevator button does not light up and Rourke appears not to actually press it; he then goes to a bank with no name other than Bank and opens a single safe-deposit box with one key (the bank's key, though your own key is also required). Near the end of the film, as Rourke leaves the therapist's building, he already knows he is in a simulation: to the left of the building's name (Occupational Therapy Department) is a double-D logo that was not present in the same scene at the start of the film. That logo can be seen throughout the real facility, meaning he is already beginning to see the truth.
