The naked businessman tied to the bed at the start of the film is one of Malcolm's identities; he is also the agent Caroline Suzanne speaks to on the phone. He was written as the first identity to die in the film, but those scenes were cut.

A life-size dummy was created to depict the murdered character played by Jake Busey with a baseball bat driven through his throat. One of the studio executives wanted to keep the dummy as a souvenir and stored it in his office closet. One night a cleaning woman opened the closet and was horrified. After the incident, the dummy was immediately removed from the offices.

Identity has several connections to Alfred Hitchcock and his film Psycho (1960). First, the film is largely set in a motel resembling the Bates Motel. Second, as in Psycho, it was raining. Third and biggest: the shower curtain scene. Hitchcock's classic has rain falling on the first victim; the curtain rod is torn from the wall. In Identity, the curtain is torn down and used against the rain outside, and the first victim dies.
