Director Jon Favreau wanted Robert Downey Jr. because he thought the actor's past suited the role. He commented: "Robert's best and worst moments in life were in the public eye. He had to find an inner balance to overcome obstacles that went far beyond his career. That's Tony Stark. Robert brings a depth that goes beyond a comic book character who had trouble in high school or can't get the girl." Favreau also felt Downey could make Stark a "lovable asshole" while also portraying a genuine emotional journey as he wins over the audience.

Brian Michael Bendis wrote three pages of dialogue for the Nick Fury scene, from which the producers selected the best lines. To keep it a secret, the scene was shot with a core crew and cut from all of the film's previews, preserving the mystery and surprise and keeping fans speculating and engaged. It ultimately appeared as a post-credits scene in the final cut and started the tradition of every film in the Marvel Cinematic Universe having at least one post-credits or mid-credits scene.

This was the only film in which Terrence Howard played Lieutenant Colonel James "Rhodey" Rhodes. Don Cheadle was brought in to take over the role starting with Iron Man 2 (2010). Howard was reported to have been the first actor cast in Iron Man and also the highest-paid actor on the film set (given that Robert Downey Jr. was not a reliable star at the time). He was promised a three-film deal, but claimed he was dropped from the sequel after the studio failed to honor his contract. Although Marvel never officially commented on the matter, insider information indicated that director Jon Favreau was unhappy with Howard's performance, which often required reshoots, and that this was why the role was recast. Howard later admitted that he turned down the offer because he was offered a much lower salary to accommodate Robert Downey Jr.'s rising pay.

In March 2005 they published a revised list: Oded Fehr as Iron Man, Yasiin Bey as James Rhodes, Rachel McAdams as Pepper Potts, Eva Longoria as Madame Masque, Cary-Hiroyuki Tagawa as the Mandarin and Dolph Lundgren as Crimson Dynamo. McAdams was actually Jon Favreau's first choice to play Pepper, but she turned it down.

Ahmed Ahmed had played terrorists for many years and eventually asked his manager to stop offering him such roles because he was uncomfortable perpetuating harmful stereotypes. Nevertheless, his manager submitted him for a terrorist role in this film, and he deliberately played it "over the top" at the audition in order to avoid being cast.
