Morjana Alaoui, the actress who played Anna, broke three bones in her foot when she fell 3 metres (9.8 ft) from the set during filming. She had to stay in bed for six weeks, and production was temporarily halted for more than a month.

Mylène Jampanoï and Morjana Alaoui, who played Lucie and Anna, accepted their roles after being affected by the script; Jampanoï even took on the role despite her agent's reservations. While preparing for the shoot, the actresses rehearsed with director Pascal Laugier for two months, and during filming Laugier isolated them from the rest of the crew to keep them in a heightened emotional state. In an interview, both said they found the shoot so emotionally difficult that they could never work with the director again, and Jampanoï recalled: "Every night when I went back to my room I just cried because I was so physically and psychologically exhausted. All my scenes are violent." Though she respected Laugier's working style, she found him to be "as temperamental as I am… I have great admiration for him… but in the end we clashed."

North American distribution rights were acquired by The Weinstein Company, but studio owner Bob Weinstein was so horrified by the film that he skipped a theatrical release and put it out directly on DVD through a partner company, both in an unrated edition and in a heavily edited, R-rated version. The film was later remade as Martyrs (2015) by producer Jason Blum's Blumhouse Productions, something Blum later regretted. Pascal Laugier, director of the French original, said that selling the remake rights was the only regret of his career, because he hated the American version and, due to a bad contract negotiation, never received any money from it.
