Needful Things is set in the fictional town of Castle Rock, Maine, as in Stephen King's novel. The distribution company Castle Rock took its name from it when it released Stand by Me (1986). Castle Rock was also the setting of The Dead Zone (1983), Cujo (1983) and The Dark Half (1993). It was also the setting of Stand by Me.

The car Leland Gaunt drives is a 1958 Mercedes-Benz 300d (W189). At the time it was the company's flagship model. It was equal to its rival, the Rolls-Royce Silver Cloud, in features and price, but superior in performance. Favored by statesmen and by the embodiment of immortal evil, it offered options such as a glass partition, a VHF car phone and a dictation machine.

At the behest of the TBS network, Fraser C. Heston prepared a two-part miniseries cut containing more than an hour of material cut from the film's theatrical version. This version was presented under the title "More Needful Things." Despite Heston's involvement, Heston does not consider this version a Director's Cut. This version was not released on video in the US, but appeared as an extra on the German Blu-ray. (before 2023).

Bonnie Bedelia had previously appeared in "Salem's Lot" (1979 TV miniseries), also adapted from a Stephen King novel and likewise about a mysterious stranger who moves to a small town and opens an antique shop. That production also featured two actors who were married in real life (James Mason and Clarissa Kaye-Mason), as this film does (Ray McKinnon and Lisa Blount). In both, an actor appears whose spouse appeared in the same characters' other film. Julie Cobb appeared in "Salem's Lot," in whose 2004 remake her husband James Cromwell appeared. Ed Harris's character had previously appeared in The Dark Half (1993), in which his wife Amy Madigan had also appeared.

At one point in the novel, Gaunt is seen reading Treasure Island and later quotes from Hamlet. Director Fraser C. Heston directed his father, Charlton Heston, in Treasure Island. The elder Heston also played the Player King in Hamlet (1996). Amanda Plummer's father, Christopher Plummer, himself portrayed Hamlet.
