Michael Caine agreed to appear in the film only if the director also cast Denholm Elliott. Caine had known Elliott since they appeared together in Alfie (1966), 26 years earlier, and Elliott had told Caine that he had AIDS and did not have long to live. Knowing this was probably Elliott's last chance for a paycheck, Caine insisted on his inclusion. Elliott died just a few months after the film, which deals with the complications of AIDS, was released in theaters.

The play on which the film is based consists of three acts: Act 1 is the final dress rehearsal in January, Act 2 is a performance some time later, and Act 3 is a performance in April. The bridging scenes with voice-over narration, including the opening scene in which Sir Michael Caine leaves the theater and the closing scene in which he returns, were written for the film and do not appear in the play. The play's final line is a frantic call for "Curtain!"
