In an alternate scene, Alexandra Daddario plays a small female role as a scuba diving partner in a sequence featuring another giant squid-like monster. The entire sequence was cut and never made it into the original theatrical version. The scene was made available in the special features of the blu-ray release.

Davis Okoye mentions in the film that George is the only albino gorilla in existence. In reality, there was only one known albino gorilla. He was born in Spanish Guinea around 1964 and was brought to the Barcelona Zoo in 1966. The mayor of Barcelona named him "Blancanieves" (Snow White), but a year later, when he appeared on the cover of National Geographic magazine, he received his final name, "Snowflake." During his time at the Barcelona zoo, Snowflake fathered twenty-two children, but only six survived to adulthood. None of his offspring were albino. He died at the Barcelona Zoo on 24 November 2003 at the age of 38.

At the beginning of the film, while Dr. Atkins is collecting samples, he takes only 3 of the boxes, and when the rodent creature chases him, it appears that 5 are left in the cabinet. Later in the film, Claire Wyden states that the 3 boxes he took survived re-entry because they were designed that way. The other 5 remaining boxes could still hit Earth.

The monsters in the 'Rampage' video game series on which this film is generally based were mutated humans (who had turned into giant animals); here they are mutated animals. Genetic engineering causes their size to grow, their aggression to increase, and them to adopt some genetic traits from other species (such as certain regenerative abilities).

George is bipedal in the games but walks on his knuckles like a normal gorilla. Ralph is a giant werewolf in the games, so he naturally stomps around on his hind legs like a human. Because he is a mutated wolf in this film, he stands on all fours. Lizzie is quadrupedal like a normal crocodile, whereas in the games she walked on her hind legs. This change may have been made to keep Lizzie from parodying Godzilla as much as she did in the games.

Ralph is a wolf with bat ears, porcupine quills and flying squirrel membranes, while Lizzie is a crocodile with wild boar tusks, fish gills and lizard toes, as well as pangolin-like armor and a spiked tail club. George is the odd one out, since he is basically just an albino King Kong. Technically speaking, however, this trope applies to all three because of genetic editing, as they are also filled with the indeterminate growth of the great white shark, the growth rate of the blue whale, the strength of the rhinoceros beetle, the speed of the cheetah, and the tissue regeneration of the African spiny mouse.

At the end of April 2018, Uwe Boll demanded that Warner Bros. change the name of the film about to be released, or he would sue the studio. The director, who had produced and directed an unrelated "Rampage" trilogy starting with Rampage (2009), claimed that the studio would "shrink" his brand and the revenues he could use for future installments of his own "Rampage" series.

The film is based on the Rampage video game (published by Bally Midway), in which players take control of giant monsters trying to survive attacks by military forces. The game's heroes and playable characters are George, a giant King Kong-like gorilla transformed by an experimental vitamin; Lizzie, a Godzilla-like dinosaur/lizard transformed by a radioactive lake; and Ralph, a giant werewolf transformed by a food additive.

Every branch of the US armed forces except the Coast Guard took part in the attacks on the monsters. A pair of Navy F/A-18 fighter jets escorts the Air Force bomber; this is unusual and seems unnecessary on the continental United States, but may have been done to give the Navy some nominal involvement.

The antidote nicknamed the "chill pill" works on several levels: it treats those affected by the mutagen whose side effect is increased aggression, meaning it is a drug that literally calms them; it is stored in a vat of liquid nitrogen, so it is literally chilled; and, as Claire later discovers, it is administered orally like a real pill.
