The web series originated from a post published on 4chan by an unknown user on May 12, 2019, which was based on a photograph first taken by Bill Magritz in 2002 in an old store called Rohner's Furniture in Oshkosh, Wisconsin. In the 2019 post, the anonymous user accompanied the photo with a text: "If you're not careful and you noclip out of reality in the wrong areas, you'll end up in the Backrooms, where it's nothing but the stink of old moist carpet, the madness of mono-yellow, the endless background noise of fluorescent lights at maximum hum-buzz, and approximately six hundred million square miles of randomly segmented empty rooms to be trapped in. God save you if you hear something wandering around nearby, because it sure as hell has heard you." Noclipping is a video game glitch that disables collision detection, allowing a player or object to move freely through solid barriers such as walls, floors and ceilings and to access sections of the game that were not programmed to be there. Kane Parsons based the idea of Backrooms on this image-text combination.

Through different episodes, it is revealed that on May 10, 1982, Oak Ridge National Laboratory experimented on a prototype of the newly discovered Low Proximity Magnetic Distortion System. The test successfully tore a hole in the fabric of reality, or opened a door to the endless yellow labyrinth known as the Backrooms. A year later, the A-Sync Research Institute (A-Sync for short), originally founded to manufacture MRI machines for hospitals and medical centers, was established. On June 4, 1988, A-Sync conducted the first Low Proximity Magnetic Distortion System test seeking to create "space within space" to solve the overpopulation and lack of storage space crisis that shook the US in the late 80s. On July 2, 1988, the third test succeeded but caused severe problems at the facility due to excess power, leading to the test being shut down. Finally, on October 17, 1989, the sixth test successfully opened the Backrooms threshold known as The Complex. In the sequel film Backrooms (2026) (set in 1990), it is revealed that A-Sync investigates the countless invisible portals and doors that appear everywhere and, by leading to the Backrooms, tries to discover the reason.

The Backrooms is a parallel dimension outside the known universe but very close to reality. It consists of endless interconnected interior rooms where physical laws apply strangely, there are no exterior views and no breathable air other than the sun. These rooms are usually empty, but some can contain all kinds of furniture and objects, sometimes warped while remaining faithful to the original. Kane Parsons's first draft of the Backrooms consisted of nine levels (0-8); level 0 was the starting yellow lobby and the access point between realities; deeper levels became more hostile and surreal. These levels are inhabited by the Bacteria (also known as "Lifeform"), a hostile, tall, wire-legged humanoid creature that follows and kills people. The Bacteria appears as a thin, dark, organic yet mechanical silhouette resembling a skeleton made of tangled wires or mold. There is no official number for how many levels exist. Alongside Parsons, several YouTubers who made their own videos about the Backrooms showed different levels beyond level 8.
