The mural of Roslyn's Café seen in the opening title sequence is a real café. Because the series was set in Cicely, Alaska, an apostrophe and an S were added; after the series ended, the apostrophe and S were removed from the mural. Despite its prominence in the opening title sequence, Roslyn's Café never appears in any episode.

After Rob Morrow left the series, Paul Provenza was initially hired to take over the role of Dr. Joel Fleischman. The difference in his appearance was to be explained by Fleischman getting a new haircut, which Janine Turner's character, Maggie O'Connell, "suited him." The idea was rejected to avoid alienating Morrow's fans, so Paul Provenza became a new doctor named Phillip Capra. The series lasted only eight more episodes after Morrow's departure.

In some episodes, a calendar for "Clicquot Club" ginger ale can be seen hanging in Dr. Fleischman's office. The calendar features a cartoon of a woman on a bicycle with a baby on her lap, alongside the slogan "Refreshment Coming Your Way." The calendar page is stuck on January 1942. The ginger ale was sold from the 1920s until the 1960s, when the company went bankrupt.

Wisconsin singer-songwriter Justin Vernon named his project "Bon Iver" after the episode in which people wish each other "bon hiver" (good winter) following the first snowfall. He has several times called the series his favorite (he has a tattoo of it on his left arm) and said he cried for about four hours after it ended.
