Luke Evans's character is named John Schuyler Moore. Schuyler is an old New York Dutch family whose roots reach back long before the nation's founding and which had close ties to the Roosevelt clan. One of Moore's well-known ancestors was Elizabeth Schuyler, the wife of Alexander Hamilton.

Blackwell's Island was later called Welfare Island and was subsequently renamed Roosevelt Island. Because of the number of hospitals and asylums, the area was designated Hospital Park. By the middle of the twentieth century it had become a mixed-use district with the launch of housing developments, and in the twenty-first century a satellite campus of Cornell University and Franklin Roosevelt's Four Freedoms Park were established at the island's southern end.

In The Alienist, the actual first Caleb Carr novel on which Season 1 is based, the journalist John Schuyler Moore (Luke Evans) is the first-person narrator, and Dr. Kreizler's young driver Stevie Taggert (Matt Lintz) is a secondary character. In Carr's second novel, The Angel of Darkness, on which Season 2 is based, Stevie Taggert is the first-person narrator and features in a major subplot involving an ill-fated relationship with a young prostitute named Kat. For the TV Season 2 adaptation, however, Stevie, Kat and their entire related subplot were written out completely, apart from a very brief initial conversation between Kreizler and Stevie.
