Roshan Sethi, one of the series creators, was a real physician. He currently works as a palliative radiation oncologist at Brigham and Women's Hospital, part of Harvard Medical School, where he is completing his residency. After writing the pilot with Amy Holden Jones, who appears in every episode of the series and writes several episodes each season, neither Sethi nor his partner Hayley Shore continued with the series or wrote another episode. Multiple doctors are part of the show's staff as writers.

The series was first developed in 2016 at Showtime under the title "The City." It was then turned around (Hollywood parlance for when a studio decides not to develop a project further and offers it to other studios to cover development costs), and the following year Fox bought the series from Showtime. The original concept centered on an intern who gradually learns that his superior, a newly arrived senior resident at the hospital, is a murderous psychopath. This was dropped when the series moved to Fox.

On Gilmore Girls, Czuchry played Logan Huntzburger, who has a disjointed relationship with his quite wealthy father, works as a newspaper owner and pushes his son to work in print journalism.
