This game, a tribute to the classic Marvel series "What If?", includes a "What If?" mode presented by the Watcher (as in the original comics) and accessed through a password screen. With the mode active, you can play the game and observe the (mostly comical) changes that occur. The changes range from trivial alterations to complete departures from the original premise. The mode is not available in the Nintendo 64 version.

The PlayStation's hardware limitations made creating a more detailed version of New York impossible. To mask this, a fog effect was added to the ground, and the developers decided to make it part of the story.

During the "What If?" mode, other Marvel characters can be seen in the background, including the Watcher (who introduces the "What If?" mode), the Silver Surfer (when Spider-Man is chased by the police and rides the exploration balloon), Ghost Rider (seen very briefly riding down the side of a building at the very start of the "Race to the Bugle" level) and the Sub-Mariner (seen outside during the underwater battle with Carnage). The Sub-Mariner even has an entry in the game's character database.

In some versions of the game (but not the Dreamcast version), entering "RULUR" as a cheat code causes J. Jonah Jameson to be replaced throughout the game by Neversoft's own J. James Jewett (who also has an entry in the game's character database).

Monster-Ock was originally named Super Ock and had a different design from the final version. Initially Monster-Ock had sunglasses, red skin, orange clothing and gold-colored mechanical arms. In the final version the design was changed substantially: the sunglasses were removed and its appearance was redesigned to resemble Carnage. In the final game, however, Monster-Ock's icon still shows it with sunglasses while it chases Spider-Man.
