At the end of her 17-year relationship with Jack Nicholson, Nicholson gave her a pearl and diamond bracelet that had once been a gift from Frank Sinatra to Ava Gardner; this was how Nicholson ended his relationships. Huston appeared in five films with Nicholson: One Flew Over the Cuckoo's Nest (1975), The Last Tycoon (1976), The Postman Always Rings Twice (1981), Prizzi's Honor (1985) and The Crossing Guard (1995).

She survived a nearly fatal car accident. Driving down Coldwater Canyon at dusk, a BMW came toward her too fast and hit the bumper of the car in front of her. Huston's face struck the windshield and she was admitted to Cedars-Sinai, where she underwent a long operation to remove bone fragments from her forehead and skull and to reconstruct her nose. When she woke up, Jack Nicholson was there with flowers. In an interview, Huston shared that the accident changed her life.

Contrary to what many people assume, her childless status was not by choice. In a 1998 episode of Lifetime Intimate Portrait (1990), she said she had tried several times to have a baby. Elaborating on the subject in her autobiographies, Huston wrote that she was unable to conceive with her husband Robert Graham as well as with her former boyfriends Bob Richardson and Jack Nicholson. She began trying to conceive with Richardson while still a teenager and continued trying occasionally into her mid-forties, even undergoing IVF treatment.

Marsha Mason and Richard Dreyfuss presented her with the Oscar. After her speech, with tears streaming down her cheeks, she left the stage and went into the audience to stand for her father, who was seated in the middle of the middle row. Tzetzi Ganev designed her green silk Oscar dress in honor of her upbringing in Ireland.
