Four gangs were created that did not make it into the game: the Camheads, the Clowns, the Lost and the Jury. The Camheads wore suits and had cameras mounted on their heads; the Clowns were dressed as their name suggests; the Lost wore cleaned-up metal; and the Jury wore hockey masks. The hockey mask design ultimately went to the Skinz gang. The Clowns were given a backstory with obvious parallels to serial killer John Wayne Gacy, including a lawyer who shared Gacy's surname. The Lost motorcycle club of Grand Theft Auto IV: The Lost and Damned (2009) served as a tribute to the scrapped gang from Manhunt.

The game's concept came about when Rockstar Games president Sam Houser asked writer Christian Cantamessa to find a horror story that did not involve monsters, vampires or fantasy characters. Artist Alan Davidson drew concept art for the game; one of the drawings was an underpass in which a character is chased by a gang, with the main character barely visible and only the shadows of the people chasing him in view. This reminded Christian of the film A Clockwork Orange (1971), and he thought that having players be chased, as the homeless man in the film is chased by the Droogs, would be a frightening concept.

Two "boss" characters did not make it into the game: Korkuluk and Binbag. Korkuluk was to be the former leader of the Smileys, stripped of his title after being defeated by Piggsy, with whom he carried on a feud. When not in his costume, Korkuluk was nothing more than a drunk vagrant spouting meaningless words, who can be seen in the game when Cash confronts the Innocentz. Binbag was to be the leader of the Lost (an unused gang) and would have been dressed head to toe in black garbage bags.
